Application forms for the Tuition Assistance
Program
We must receive and approve two different forms
before you may begin a course funded by the Tuition
Assistance Program:

Application Form
First, you must complete and send us a Tuition
Assistance Program Application Form to find out
whether you are eligible for Tuition Assistance
Program funding.
The application form is available for download in
either PDF or
MS Word format.

If you are not able to download or print the form,
please call the Community Development Trust toll
free at 1-877-238-8882 and we will arrange for you
to receive a copy.
In addition to information such as your name,
address and birth date, you will need the following
information to apply:
- Your social insurance number
- The date you started working in the forest
sector
- The name of your employer and their contact
information
- Your job title
- When you were laid-off
Along with your application, please send us copies
of any documents you have that confirm this
information. These may include your Record of
Employment (ROE), layoff notice, Canada Revenue
Agency T4 slips, a T1 form with business activities
statement, contracts, and/or correspondence showing
your employment history.
When complete, please mail your application along
with copies of supporting documentation to:

If we find that you meet the program’s criteria, we
will then send you a letter of eligibility to take
to your school’s Financial Aid and Awards Office.
School Information Form
Once you have registered for your course(s) and
the School Information Form is completed by the
Financial Aid and Awards Office, all original
documents need to be sent to us. If the information
is complete and accurate, it will take from
ten to 15 business days
for a cheque to be issued. Please make sure you
allow enough time for your information to be
processed and for the payment to be received by the
school, before your course
