Direction to Pay

This form provides us with the information we need to pay you your Transitional Assistance Program funding. Before beginning to fill out the form, please make sure you have made decisions on the points covered in our Once You Are Approved page.

Please make sure you know how and when you want to receive your funding before filling out this form. We are unable to accommodate changes once your Direction to Pay form has been processed.

To fill out the Direction to Pay form, please follow these five steps:

  1. Check the personal information that is printed at the top of the form, including your name, Social Insurance Number (SIN) and the total amount of Transitional Assistance Program funding you are eligible to receive (which should match the amount indicated in our letter to you).
  2.  Enter your resignation/retirement date on the line below the personal information and make sure it is the same as the resignation/retirement date you enter on the other two forms. Please remember that your resignation/retirement date is not the same as your lay-off date.
  3. In the Cash section of the form, enter the total amount of your benefit that you wish to receive in cash and below that, how much of it you wish to receive in each of the two time periods listed. It is entirely up to you to decide how much of your funding to receive during each payment period.
  4.  In the Registered Retirement Savings Plan (RRSP) Contributions section of the form, enter the total amount of the benefit that you wish to have deposited directly to your RRSP and below that, how much of it you wish deposited to your own and/or a spousal and/or common-law RRSP, in each time period listed. The total amount in all four boxes should equal the amount you have entered as the total to be paid into RRSPs.

    Please check carefully to make sure the RRSP account number(s) you write in are correct. Have the financial institution apply its address stamp in the place indicated for each RRSP you wish to receive deposits. This is to make sure we deposit your funding with the correct financial institution.
  5.  At the bottom of the form, sign and write the date.

You must send us your original form with your signature on it for us to be able to process your payment.
