Transitional Assistance Program
Declaration Form
On this form, you must sign eight times to indicate
your commitments to the Transitional Assistance Program.
You must agree to all the points
listed on the form in order to participate in the
program. Make sure you
sign all eight sections, even if you feel the
information there is not relevant to your situation. You
may indicate this by writing ‘not applicable’ beside any
of the statements that do not apply, but you still must
sign beside each statement, indicating that you have
read, understood and agreed to it. At the top of the
form, you will see your name and Social Insurance Number
(SIN). Please check carefully to make sure this
information is correct. Beneath the identification
information, there are eight statements listed. Please
read them carefully and sign beside each of the
statements. The eight sections confirm the following
information:
- The information you have provided to us is
complete, accurate and true.
- You have provided us with information about any
severance payments.
- You understand that Transitional Assistance
Program funding affects Employment Insurance (EI)
eligibility.
- You consent to us informing Service Canada of
your resignation/retirement.
- Confirmation that you will resign/retire
on your selected date. Enter your
resignation/retirement date in the box containing
this statement and make sure it
is the same as the resignation/retirement date you
enter on the other two forms. Please remember
that your resignation/retirement date
is not the same as your
lay-off date.
- You agree not to return to work for your
employer, a subsidiary company or on the same site
for 18 months.
- You agree to contact us and cooperate if your
circumstances change and you return to work contrary
to program rules.
- You understand that if you provide false
information or break program rules you will be
required to return funds received, plus interest and
penalties.
At the bottom of the page, enter the date.
You must send us the original
form with your signatures on it for us to be able to
process your payment.
